Housing First for the Chronically Homeless
The Matt Urban Housing First program provides 68 units of scattered site housing to the chronically homeless. This low-demand model focuses of the recovery from the homelessness and housing stability.
All of our Housing First residents have an intensive case manager who works with them to improve their physical health and well-being, and assist them with housing needs.
A chronically homeless individual is defined by HUD as an unaccompanied adult with a long term disability who has been homeless for one year or more, or has experienced four episodes of homelessness in a three year period
