Senior Services Case Manager (Part-time)

A Part Time Case Manager is responsible for assisting clients, age 60 and over, with maintaining their independence in their own home, and avoiding placement in assisted living or a nursing facility.  Candidate will link clients to benefits/entitlements, home delivered meals, housekeeping, personal care, social adult day care, and/or personal emergency response systems.

Hope Gardens Case Manager

The Hope Gardens Case Manager will maintain a client case load and work as a team to provide services to the individuals of Hope Gardens, a 20-unit permanent supportive housing site for chronically homeless women.

Rapid Re-Housing Case Manager

The Rapid Rehousing Case Manager’s primary responsibility is to administer the Rapid Rehousing Program according to the Department of Housing and Urban Development’s (HUD) regulations and with the City of Buffalo’s Policies and Procedures. Additional major responsibilities include providing case management services to clients enrolled in the program, and assisting clients in obtaining permanent housing.

Housing First Case Manager

The Housing First Case Manager will know and understand the Housing First model, the definition of chronically homeless and how to document/verify. They will work with homeless clients to perform thorough assessments, complete Independent Services Plans, and work toward client goals while updating and tracking progress.

Weatherization Energy Auditor

The Energy Auditor’s responsibilities will be to inspect customer’s homes before and during the Weatherization work, to perform an energy audit and develop a work scope and perform a final Quality Control post inspection.

Weatherization Crew Laborer

Laborers will work on client’s dwellings according to auditor’s work scope. The crew shall do all work assigned by the Crew Chief or the Energy Auditor. The crew laborer will know and abide by Standard Work Specification guidelines.
