The Community Health Worker will work with residents and instruct them on activities of daily living, including cleaning their apartment, appropriate personal hygiene, etc. The CHW will develop creative ways to foster community building and social development.  They will provide transportation to clients as needed. The Community Health Worker (CHW) has the primary responsibility for assisting all in-house clients and the team during their respected shift. This individual renders brief individual and family interaction as well as some case management and service linkage coordination. Housing AmeriCorps Member – Full Term

The AmeriCorps Service member will work with housing staff to ensure our housing department is able to meet the continued needs of our community and residents. Their support will assist with a high volume of housing related intake for the various programs within the department, ranging from low income housing, homebuyer and rehab programs and tenant related referrals.

The Homeless Outreach Case Manager is a field position that has the primary responsibility of successful engagement of street homeless and chronically homeless individuals in the City of Buffalo, assisting them with basic needs followed by housing.
